Metformin is not FDA-approved for weight loss, so there is no official weight-loss dose. In studies showing modest weight change, metformin doses often ranged from 1,500 to 2,550 mg per day and were increased slowly from lower labeled diabetes doses to reduce stomach side effects. A licensed clinician sets any dose based on kidney function, tolerance, and goals 1, 2, 3, 4.

Is metformin approved for weight loss?

Metformin is FDA-approved for type 2 diabetes, not weight loss. When it is used to support weight management, that use is off-label, meaning it is outside the FDA-approved indication on the label 1.

FDA-approved uses vs. off-label use

Metformin hydrochloride, sold under brand names such as Glucophage, Glumetza, Fortamet, and Riomet, belongs to the biguanide drug class. The FDA label describes its approved use as an adjunct to diet and exercise to improve blood sugar control in people with type 2 diabetes 1.

Why clinicians prescribe it off-label

Clinicians may consider metformin off-label when insulin resistance, prediabetes, PCOS, or antipsychotic-associated weight gain is part of the picture. Studies suggest weight change is usually modest, and the same evidence shows trade-offs, including gastrointestinal side effects, vitamin B12 concerns, and kidney-related safety limits 2, 8, 9, 10.

What dose of metformin is used for weight loss in studies?

There is no official metformin dose for weight loss because this is not an FDA-approved use. In research, metformin doses around 1,500–2,550 mg per day have been studied, but those numbers describe study designs, not instructions for an individual person 2, 3, 4, 5.

Diabetes Prevention Program: 1,700 mg per day

In the Diabetes Prevention Program, adults at high risk for diabetes were assigned to metformin 850 mg twice daily, totaling 1,700 mg per day, or to lifestyle intervention or placebo. The metformin group had modest average weight loss, while the lifestyle group had greater average weight loss; individual results varied, and side effects affected some participants 2.

Real-world studies: up to 2,500 mg per day

In a real-world outpatient study, Seifarth and colleagues reported metformin doses up to 2,500 mg per day in people with overweight or obesity who did not have diabetes. The authors found greater average weight loss in people with more insulin resistance, but the study was observational and tolerability still mattered 4.

Meta-analysis findings on dose and BMI

A meta-analysis of randomized trials found that metformin was linked with small body mass index, or BMI, reductions in several groups, including people with obesity and some with insulin resistance. Results varied across studies, and side effects such as diarrhea and nausea were common reasons metformin was harder for some people to tolerate 5, 7.

How is metformin titrated up to the target dose?

Metformin is often increased slowly in clinical care because stomach side effects are dose-related. For FDA-labeled diabetes treatment, the label describes starting at 500 mg twice daily or 850 mg once daily for immediate-release metformin, with gradual increases; this label dosing is not a weight-loss instruction 1.

Starting dose: 500 mg in FDA labeling

The FDA label for immediate-release metformin describes 500 mg twice daily as one starting schedule for adults with type 2 diabetes. That number comes from diabetes labeling and should not be read as a personal weight-loss dose 1.

Weekly increases in FDA labeling

For immediate-release metformin used for type 2 diabetes, FDA labeling describes dose increases in 500 mg weekly steps or 850 mg every two weeks, as tolerated. Slow increases are used because nausea, diarrhea, gas, and abdominal discomfort are common metformin side effects 1, 7.

Typical maintenance range in labels and studies

For FDA-labeled diabetes use, maximum total daily doses differ by product and formulation, and some immediate-release labels list up to 2,550 mg per day in divided doses. In weight-related studies, commonly reported totals were lower or similar, but any target dose must be individualized by a clinician after safety screening 1, 2, 4.

Immediate-release vs. extended-release: which is used for weight loss?

Both immediate-release and extended-release metformin are used in clinical care, but weight management remains off-label for both. Extended-release metformin may be considered when stomach side effects limit immediate-release use, though the choice depends on the person, product, kidney function, and clinician judgment 1, 7.

Dosing schedules

Immediate-release metformin is usually labeled for divided daily dosing in type 2 diabetes. Many extended-release products are labeled for once-daily dosing with the evening meal for type 2 diabetes, but product labels differ 1.

Tolerability differences

A systematic review found that gastrointestinal adverse events are common with metformin and may be less frequent with extended-release formulations than immediate-release formulations in some studies. Extended-release metformin can still cause nausea, diarrhea, and abdominal discomfort, so monitoring is still important 7.

Available strengths

Metformin hydrochloride products are available in several tablet and liquid forms, including immediate-release tablets, extended-release tablets, and oral solution products such as Riomet. Exact strengths and maximum labeled doses depend on the product and manufacturer 1.

How much weight can you expect to lose, and how fast?

Metformin-related weight loss is usually modest, and it is not guaranteed. In the Diabetes Prevention Program, average weight change was smaller with metformin than with intensive lifestyle intervention, and side effects were part of the risk-benefit picture 2, 3.

Timeline: 3, 6, and 12 months

When weight change happens with metformin, it usually happens over months rather than days. DPP and follow-up data support slow, modest weight change, with durability depending in part on adherence and ongoing lifestyle habits; individual results vary 2, 3.

Who tends to lose the most weight

People with insulin resistance may be more likely to have weight change with metformin, based on observational data and metabolic-study findings. That does not mean metformin is right for every person with insulin resistance, because kidney function, pregnancy status, alcohol use, drug interactions, and GI tolerance can change the safety profile 1, 4, 6.

Weight regain after stopping

Long-term DPP follow-up suggests weight benefits were more durable among participants who continued metformin and were adherent. Stopping any weight-related treatment can be followed by weight regain, but the exact pattern varies by person and by the lifestyle plan used alongside treatment 3.

Does the dose need to be adjusted for kidney function or other conditions?

Yes. eGFR below 30 mL/min/1.73 m² is a key safety cutoff in FDA labeling because metformin is cleared by the kidneys and carries a boxed warning for lactic acidosis 1, 6.

eGFR thresholds

FDA labeling says metformin is contraindicated when eGFR is below 30 mL/min/1.73 m². FDA guidance also states that starting metformin is not recommended when eGFR is between 30 and 45 mL/min/1.73 m², and kidney function should be checked before use and at least yearly in many patients 1, 6.

PCOS and prediabetes considerations

For PCOS, metformin has been studied for metabolic features such as insulin resistance and cycle-related outcomes, but it is not FDA-approved for PCOS weight loss. International PCOS guidance notes metformin may be considered for metabolic outcomes in selected adults with PCOS, with attention to side effects and shared decision-making 9.

For prediabetes, the Diabetes Prevention Program studied metformin 850 mg twice daily and lifestyle intervention in people at high diabetes risk. This research supports modest weight and metabolic effects in the study population, but it does not create an FDA-approved weight-loss indication 2.

Antipsychotic-induced weight gain

Metformin has also been investigated for antipsychotic-associated weight gain. A meta-analysis found metformin was linked with weight and metabolic improvements in some people taking antipsychotics, but use requires careful medication review because psychiatric stability, drug interactions, and GI side effects matter 8.

What are the side effects at higher doses?

Higher metformin exposure can make stomach side effects more likely, especially during dose increases. The most common issues are diarrhea, nausea, vomiting, gas, indigestion, abdominal discomfort, and appetite changes; lactic acidosis is rare but serious and is highlighted in the boxed warning 1, 7.

GI side effects

The FDA label lists gastrointestinal reactions as common with metformin. A systematic review also found increased gastrointestinal adverse events compared with controls, which is why clinicians often use gradual titration and may consider extended-release products when appropriate 1, 7.

Vitamin B12 monitoring

Long-term metformin use has been associated with lower vitamin B12 levels. The DPP Outcomes Study reported biochemical B12 deficiency was more common in metformin users than placebo users over long-term follow-up, so clinicians may monitor B12, especially if anemia, nerve symptoms, or long-term use is present 10.

Lactic acidosis boxed warning

The metformin label carries a boxed warning for lactic acidosis, a rare but life-threatening buildup of lactate. Risk is higher in settings such as significant kidney impairment, acute illness, dehydration, heavy alcohol use, certain imaging procedures with iodinated contrast, liver disease, or low-oxygen states; FDA labeling and safety communications guide when metformin should be avoided or held 1, 6.

How does metformin compare to GLP-1 medications for weight loss?

Metformin and GLP-1 medicines work differently and have different roles in weight care. Semaglutide 2.4 mg once weekly is the active ingredient studied in the STEP 1 weight-management trial, while metformin’s weight-related use is off-label and usually produces smaller average changes 1, 11, 13.

Metformin: insulin signaling and liver glucose output

Metformin is a biguanide that mainly lowers glucose production by the liver and improves insulin sensitivity. That mechanism can be useful when insulin resistance is part of the health picture, but the weight effect tends to be modest and must be balanced against GI side effects, B12 deficiency risk, kidney limits, and the boxed warning for lactic acidosis 1, 2, 10.

Semaglutide: GLP-1 receptor signaling

Semaglutide, sold as Ozempic for type 2 diabetes and Wegovy for chronic weight management, is a GLP-1 receptor agonist; compounded semaglutide may also be prepared for patient-specific prescriptions by licensed 503A pharmacies. GLP-1 signaling helps regulate appetite, stomach emptying, and glucose-dependent insulin release; in STEP 1, participants receiving semaglutide 2.4 mg once weekly had greater average weight loss than placebo, with gastrointestinal side effects common and individual results varying 11, 13.

Tirzepatide: GIP and GLP-1 receptor signaling

Tirzepatide, sold as Mounjaro for type 2 diabetes and Zepbound for chronic weight management, activates both GIP and GLP-1 receptors; compounded tirzepatide may also be prepared for patient-specific prescriptions by licensed 503A pharmacies. In SURMOUNT-1, participants receiving once-weekly tirzepatide at studied doses of 5 mg, 10 mg, or 15 mg had greater average weight loss than placebo, with gastrointestinal side effects common and individual results varying 12, 14.

When each is considered

A clinician may consider metformin when insulin resistance, prediabetes, PCOS, type 2 diabetes, or antipsychotic-associated weight gain is relevant 1, 2, 8, 9. GLP-1 or GIP/GLP-1 options may be considered when a person meets weight-management criteria, but contraindications and side effects must be reviewed, including thyroid C-cell tumor warnings on semaglutide and tirzepatide weight-management labels 13, 14.
