Metformin is not FDA-approved for weight loss, so there is no official weight-loss dose. In studies, common daily doses are 1,000–2,000 mg, often after starting at 500 mg and increasing gradually under clinician care. Average weight loss is modest, often about 3–6% over 6–12 months, and results vary [1,2].

Is metformin approved for weight loss?

Metformin is not FDA-approved for weight loss. It is FDA-approved as metformin hydrochloride, a biguanide drug class medication, to improve blood sugar control in adults and children with type 2 diabetes when used with diet and exercise [1].

Brand names include Glucophage and Glucophage XR, and other metformin products have included Fortamet, Glumetza, and Riomet. Immediate-release metformin and extended-release metformin work on the same active drug, but their release pattern and dosing frequency differ [1].

FDA-approved uses vs. off-label use

The FDA label is for type 2 diabetes blood sugar control, not weight loss [1]. However, metformin has been studied in people with prediabetes, insulin resistance, and PCOS, where weight changes may be tracked as part of broader metabolic outcomes [2,3,5].

Who typically gets prescribed metformin for weight?

Clinicians may consider metformin when a person has type 2 diabetes, prediabetes, insulin resistance, or PCOS, especially if blood sugar or insulin markers are part of the picture [2,5,6]. It is not a general weight-loss drug, and it is not the same as GLP-1 pathway medications such as semaglutide or tirzepatide [7,8].

What dose of metformin is used for weight loss?

There is no FDA-approved metformin weight-loss dose. In major studies that measured weight, daily doses commonly landed in the 1,000–2,000 mg/day range, while FDA-labeled maximum doses for type 2 diabetes are higher for some immediate-release products [1,2,3].

Typical starting dose in labeled use

The FDA label for Glucophage describes an adult starting dose for type 2 diabetes of 500 mg twice daily or 850 mg once daily with meals, with gradual increases based on glycemic control and tolerability [1]. For Glucophage XR, the label describes 500 mg once daily with the evening meal as an adult starting dose for type 2 diabetes [1].

Titration schedule described in the FDA label

The Glucophage label describes increasing immediate-release metformin by 500 mg weekly or 850 mg every two weeks, up to 2,000 mg/day in divided doses, with higher labeled doses divided further for tolerability [1]. This is label information for type 2 diabetes, not a personal dosing plan for weight loss.

Common maintenance range in studies

Many weight-related and metabolic studies use metformin doses around 1,000–2,000 mg/day [2,3,5]. Higher doses are not automatically better, because side effects, kidney function, and adherence can limit what is safe or tolerable [1,4].

Immediate-release vs. extended-release: which works better for weight loss?

Immediate-release metformin and extended-release metformin contain the same active drug. There is not strong evidence that one form causes clearly greater weight loss, but extended-release metformin may be easier to tolerate for some people, which can affect consistency [1,9].

Dosing frequency and timing with meals

The FDA label recommends metformin with meals to reduce stomach symptoms [1]. Timing may differ by product, medical history, and side effects, so a clinician should set the schedule rather than a person changing it on their own.

Side-effect differences

A systematic review found that gastrointestinal side effects are common with metformin, and extended-release forms may improve tolerability for some people [9]. Still, extended-release metformin can cause side effects and is not safer for everyone [1,9].

How much weight can you expect to lose on metformin?

Metformin tends to produce modest weight loss, not the larger average losses seen with newer GLP-1 pathway medications. In the Diabetes Prevention Program, people assigned to metformin 850 mg twice daily lost an average of 2.1 kg over 2.8 years, while lifestyle participants lost 5.6 kg; individual results varied [2].

Results at 3, 6, and 12 months

Short-term weight changes are usually gradual. In metformin studies, the average effect is often measured over months, and a person may not see a clear change at 3 months even if blood sugar or insulin markers improve [2,3].

Diabetes Prevention Program long-term data

Long-term follow-up from the Diabetes Prevention Program Outcomes Study found that people who had early weight loss while taking metformin were more likely to maintain some weight loss over time [3]. This does not mean metformin guarantees weight loss; study averages do not predict one person’s result [3].

Why results are modest compared with GLP-1s

Metformin mainly lowers liver glucose production and improves insulin sensitivity [1]. GLP-1 pathway medications such as semaglutide and tirzepatide affect appetite and stomach emptying through incretin pathways, which helps explain their larger average weight-loss effects in obesity trials, along with their own side effects and contraindications [7,8,10,11].

Does the dose matter for how much weight you lose?

Dose can matter, but more metformin is not always better. Some studies use higher daily doses such as 1,700 mg/day in the Diabetes Prevention Program, but side effects and kidney function often matter more than chasing a higher number [1,2].

Dose-response evidence

Metformin has dose-related effects on blood sugar in type 2 diabetes, but weight-loss dose-response data are less clear [1,2]. In practice, tolerability often determines whether a person can stay on a study-used dose long enough for any metabolic effect to show [1,9].

Why higher is not always better

Higher metformin exposure can increase stomach side effects such as diarrhea and nausea [1,9]. The FDA label also warns about rare but serious lactic acidosis and gives kidney-function restrictions, so dose decisions must be medical decisions [1,4].

Metformin dose for PCOS, prediabetes, and insulin resistance

PCOS, prediabetes, and insulin resistance are common reasons metformin comes up in weight conversations. Metformin is not FDA-approved for PCOS weight loss or prediabetes weight loss, but it has been studied in these groups, including DPP metformin at 850 mg twice daily [2,5,6].

PCOS-specific dosing

The Endocrine Society guideline notes that metformin has been used in PCOS, especially when impaired glucose tolerance or type 2 diabetes is present, but lifestyle therapy is first-line for weight management in PCOS [5]. PCOS studies often use total daily doses in the 1,000–2,000 mg range, but this use is off-label and should be individualized [5,6].

Prediabetes dosing from the DPP trial

In the Diabetes Prevention Program, participants in the metformin group received 850 mg twice daily and had lower diabetes incidence than placebo over 2.8 years [2]. Weight loss occurred on average, but lifestyle intervention led to greater average weight loss in that trial [2].

Side effects and when a dose needs adjusting

Metformin side effects are often gastrointestinal, especially during dose changes. Safety also depends on eGFR, a kidney-function measure, plus other risks such as lactic acidosis and vitamin B12 deficiency [1,4,12].

GI side effects and titration

Diarrhea, nausea, vomiting, gas, indigestion, and abdominal discomfort are common metformin side effects in the FDA label [1]. Gradual label-described dose increases and taking metformin with meals are used to improve tolerability, but a clinician should decide how to adjust if symptoms occur [1].

Kidney function and eGFR

FDA safety communication says metformin is contraindicated when eGFR is below 30 mL/min/1.73 m2, and starting metformin is not recommended when eGFR is between 30 and 45 mL/min/1.73 m2 [4]. Kidney function should be checked before use and monitored during therapy [4].

Lactic acidosis boxed warning

The metformin label carries a boxed warning for lactic acidosis, a rare but serious buildup of lactate in the blood [1]. Risk is higher with significant kidney impairment, certain imaging contrast procedures, heavy alcohol use, liver impairment, or low-oxygen states [1,4].

Vitamin B12 monitoring

Long-term metformin use has been linked with lower vitamin B12 levels [12]. The American Diabetes Association recommends periodic B12 assessment in metformin-treated people, especially if anemia or nerve symptoms are present [13].

How should metformin be taken for the best results?

Metformin works best when it is taken consistently as prescribed, paired with nutrition, movement, and sleep habits that fit the person’s medical needs. The FDA label says metformin should be taken with meals, and dose changes should be gradual when clinically appropriate [1].

Taking it with food

Food can help reduce nausea, diarrhea, and stomach upset [1]. If side effects make it hard to stay consistent, a clinician may review dose timing, formulation, or whether metformin is still the right choice [1,9].

Morning, evening, or split doses

Immediate-release metformin is often described in divided doses with meals in the FDA label, while extended-release metformin is commonly described with the evening meal [1]. The right timing depends on the exact product, other medications, glucose patterns, and side effects [1].

Consistency and adherence

In long-term studies, people who stayed on metformin were more likely to maintain the modest weight changes seen in the trial [3]. But staying consistent should never mean pushing through severe symptoms without medical guidance [1].

Metformin vs. GLP-1 medications for weight loss

Metformin and GLP-1 pathway medications are different tools. In STEP 1, semaglutide 2.4 mg once weekly was associated with 14.9% mean body-weight loss at 68 weeks; in SURMOUNT-1, tirzepatide 5, 10, or 15 mg once weekly was associated with 15.0% to 20.9% mean weight loss at 72 weeks, with gastrointestinal side effects common in both trials [10,11].

When each is appropriate

Metformin may fit when blood sugar, prediabetes, PCOS, or insulin resistance is central to the plan [2,5,13]. FDA-approved weight-loss GLP-1 pathway medications may fit eligible people with obesity or overweight plus weight-related conditions, but they require review of contraindications, side effects, pregnancy plans, and cost [7,8].

Can metformin and GLP-1 medications be used together?

Metformin and GLP-1 medications are used together in type 2 diabetes care, and clinical trials of GLP-1 drugs often include people already using metformin [7,8,13]. For weight management, combining medications should be a clinician-led decision because side effects and glucose effects can overlap [7,8,13].
