Provider Licensure & Telehealth Disclosures

This page lists the licensed clinicians who provide telehealth services through Zerolimit Tech LLC d/b/a Chia Health ("we," "us"). Certain states require telemedicine providers to publicly disclose clinician licensure and identifying information on provider websites. This page is intended to support that transparency.

Important notes about telehealth services

• State availability: Telehealth services are offered only where permitted by law and only when a clinician is licensed/authorized for the patient's location at the time of care.
• Not for emergencies: If you think you are experiencing an emergency, call 911 or go to the nearest emergency room.
• Continuity of care: With your consent, we can share visit summaries with your primary care provider.
