Urolithin A: Evidence, Safety, Dosing Questions, and Access

Urolithin A is a small molecule made by gut microbes from ellagitannins in foods such as pomegranate and some nuts. Human randomized trials have studied it mainly for mitochondrial health, mitophagy-related biomarkers, muscle endurance, exercise performance, and immune-aging markers. It is not FDA-approved to treat disease or extend lifespan. Evidence is promising but not proof of broad anti-aging or disease-treatment benefits 1 2 7.

What it is

Urolithin A is a small molecule and gut-microbial metabolite made when certain gut bacteria process ellagitannins, a type of polyphenol found in foods such as pomegranate. In human research, it is often studied as Urolithin A or as Mitopure, a branded Urolithin A ingredient, with endpoints tied to mitochondrial-health biomarkers and muscle function 1 2.

Urolithin A, Mitopure, and ellagitannin metabolism

Ellagitannins are not the same thing as Urolithin A. They are food polyphenols that some people can convert into urolithins through the gut microbiome; a human clinical trial of pomegranate extract found ellagitannin metabolite formation and stool microbiota changes in healthy volunteers 12.

That gut-microbiome step matters because people differ in their “urolithin metabotype,” meaning they may make different urolithin metabolites after eating similar polyphenol-rich foods. Randomized crossover trials in postmenopausal women have studied polyphenol effects that differed by urolithin A and equol metabotypes, especially for lipid-related and TMAO-related outcomes 9 11.

Evidence grade for Urolithin A

Urolithin A is assigned evidence grade A because the retrieved PubMed set includes multiple human randomized controlled trials, including studies in older adults, middle-aged adults, male athletes, heart-failure patients, and immune-aging research 1 2 3 4 7 13. The grade describes study design and quantity, not whether Urolithin A works for a specific person and not whether it is safe.

Mechanism of action

Urolithin A is mainly discussed as a mitophagy-related compound. Mitophagy is the cell’s cleanup process for damaged mitochondria, and human trials have measured mitochondrial-health biomarkers rather than proving that Urolithin A makes people live longer 1 2 7.

Mitophagy and mitochondrial-health biomarkers

In older adults, a randomized clinical trial evaluated Urolithin A supplementation for muscle endurance and mitochondrial-health biomarkers 1. In middle-aged adults, another randomized trial studied muscle strength, exercise performance, and mitochondrial-health biomarkers 2.

The key point is that biomarkers are not the same as patient outcomes. A change in a mitochondrial marker may support a biological signal, but it does not prove broader anti-aging, weight-loss, or disease-treatment effects.

Why gut microbiome differences may matter

The gut microbiome may affect how much Urolithin A a person makes from food polyphenols. A randomized trial of multi-species synbiotic supplementation reported increased Urolithin A and butyrate production along with gut microbial diversity and inflammation-related outcomes in healthy adults 5.

Dietary polyphenol trials also add context, but they are not the same as isolated Urolithin A studies. DIRECT PLUS and PREDIMED research studied Mediterranean diet polyphenols, visceral adiposity, DNA methylation-assessed biological age, and cardiovascular-risk signatures; those data should not be treated as proof that isolated Urolithin A produces the same results 6 8 10.

Evidence

Urolithin A has evidence grade A, because two or more human randomized controlled trials are indexed in the supplied PubMed set 13. That is a strong design signal for research maturity, but it does not mean Urolithin A has proven benefits for every outcome people ask about.

Muscle endurance and strength outcomes

The strongest human theme is muscle-related research. Older-adult and middle-aged-adult randomized trials studied Urolithin A for muscle endurance, muscle strength, exercise performance, and mitochondrial-health biomarkers 1 2.

In male athletes doing resistance training, an 8-week randomized, double-blind, placebo-controlled study assessed Urolithin A effects on muscle endurance, strength, inflammation, oxidative stress, and protein metabolism 4. Individual results vary, and athlete data may not apply to older adults, people with chronic illness, or people using it for general wellness.

Exercise performance and recovery outcomes

Human trials have studied exercise performance in middle-aged adults and athletes, but the study titles and abstracts supplied here do not support turning those results into a guaranteed performance claim 2 4. Performance research is also highly population-specific: trained athletes, older adults, and sedentary adults can respond differently.

Inflammation, oxidative stress, and immune-aging biomarkers

Urolithin A has also been studied for inflammation, oxidative stress, and immune-aging markers. A randomized trial in male athletes assessed inflammation and oxidative stress endpoints, while another randomized placebo-controlled trial studied Urolithin A as a mitophagy inducer in relation to age-related immune decline 4 7.

These are biomarker and function studies, not proof of human lifespan extension. At Chia, we view longevity research through that lens: useful signals are worth studying, but biomarkers should not be sold as guaranteed outcomes.

Heart failure and cardiometabolic context

A randomized, double-blind, crossover, placebo-controlled trial evaluated Urolithin A in heart failure with reduced ejection fraction 3. That is condition-specific clinical research and should not be generalized to healthy people without a clinician’s guidance.

For metabolic health, the evidence is indirect. High-polyphenol Mediterranean diet trials have studied visceral adiposity, DNA methylation-assessed biological age, urinary polyphenol signatures, cardiovascular-disease risk, lipid quality, LDL oxidation, and TMAO-related questions 6 8 9 10 11. Those trials help explain polyphenol biology, but they do not prove that isolated Urolithin A causes the same effects.

What the evidence does not prove

  • It does not prove that Urolithin A extends human lifespan.
  • It does not prove that Urolithin A causes weight loss.
  • It does not prove that Urolithin A treats heart failure, immune decline, inflammation, or oxidative stress for an individual person.

How Urolithin A compares with other longevity compounds

People often compare Urolithin A with CoQ10, NAD+, NMN, metformin, and rapamycin. These compounds are not interchangeable: they are studied through different research categories, in different populations, and with different safety questions.
