Most over-the-counter testosterone boosters have limited or mixed evidence. A few ingredients, such as vitamin D when deficient, zinc, magnesium, and ashwagandha, show modest effects in some studies. Supplements are not tested like prescription drugs and cannot treat clinically low testosterone, so symptoms should lead to blood testing, not self-diagnosis.

What is testosterone and what does it do?

Testosterone is the main androgen hormone in men, though women make smaller amounts too. It helps regulate sex drive, erections, sperm production, red blood cell production, muscle, bone, mood, and fat distribution; diagnosis of low testosterone should use symptoms plus blood tests, not either one alone 1.

Your brain and testes run testosterone through a feedback loop. The hypothalamus signals the pituitary gland, the pituitary releases luteinizing hormone, or LH, and LH tells the testes to make testosterone. Follicle-stimulating hormone, or FSH, supports sperm production. This is why a proper workup often includes LH and FSH, not just total testosterone 1.

Testosterone also changes across the day. Levels are usually higher in the morning, which is why major guidelines recommend morning testing when evaluating possible hypogonadism 1, 2.

What are the signs of low testosterone?

Low testosterone, also called male hypogonadism, can cause sexual, physical, and mood symptoms. But many symptoms overlap with poor sleep, depression, thyroid disease, medication effects, alcohol use, obesity, and chronic illness, so a blood test is essential 1.

Symptoms in men

  • Lower sex drive or fewer sexual thoughts
  • Fewer morning erections or erectile dysfunction
  • Low energy or reduced stamina
  • Depressed mood, irritability, or trouble concentrating
  • Loss of muscle mass or strength
  • Increased body fat
  • Low bone density or fractures with minimal trauma
  • Infertility or low sperm count

Sexual symptoms tend to be more specific for low testosterone than general fatigue. Still, guidelines advise against diagnosing hypogonadism from symptoms alone because symptoms are not specific enough 1, 2.

When symptoms mean it is time to test

Testing makes sense when symptoms are persistent, new, or affecting daily life. The Endocrine Society recommends diagnosing hypogonadism only in men with consistent symptoms and “unequivocally and consistently low” serum testosterone, confirmed with repeat morning testing 1.

A common mistake is treating a single low result as the full answer. Illness, poor sleep, calorie restriction, opioids, and lab timing can shift testosterone. A clinician can decide whether to repeat testing and whether to add free testosterone, LH, FSH, prolactin, thyroid tests, iron studies, or sleep apnea screening 1, 2.

What is a testosterone booster?

A testosterone booster is usually a supplement marketed to raise testosterone, libido, strength, or energy. These products often combine herbs, vitamins, minerals, amino acids, and hormone-like compounds, but they are not the same as prescription testosterone replacement therapy.

In the United States, dietary supplements do not need FDA approval for effectiveness before being sold. The FDA can act against unsafe or misbranded supplements, but the pre-market standard is very different from prescription drugs 3.

Supplements vs. testosterone replacement therapy (TRT)

Testosterone replacement therapy, or TRT, is prescription testosterone used for men with confirmed hypogonadism. It can be given as gels, injections, patches, pellets, or other FDA-approved forms, but it also has risks, including infertility, acne, fluid retention, breast tenderness, increased red blood cell count, and prostate monitoring needs 1, 4.

Do testosterone boosters actually work?

Testosterone booster results depend on the ingredient, dose studied, baseline health, and whether a deficiency exists. The honest answer is that a few ingredients show modest signals in human studies, but many popular blends do not have strong evidence, and none should be used to treat confirmed hypogonadism without medical care.

Ashwagandha

Ashwagandha, or Withania somnifera, is an herb studied for stress, sleep, fertility markers, and hormones. In an 8-week randomized trial of overweight men ages 40 to 70, an ashwagandha extract was associated with higher DHEA-S and testosterone compared with placebo, but the study was small and does not prove it treats hypogonadism 5.

Side effects can include stomach upset, sleepiness, diarrhea, and rare liver injury reports. It may also interact with sedatives, thyroid medications, immunosuppressants, and some autoimmune conditions, so it is not risk-free 6.

Vitamin D

Vitamin D is a hormone-like nutrient linked to bone, muscle, immune function, and reproductive health. In one randomized trial, overweight men with low vitamin D who received vitamin D for 1 year had increased total testosterone compared with baseline and placebo, but this does not mean vitamin D raises testosterone in people who are not deficient 7.

Too much vitamin D can cause high calcium, kidney stones, nausea, weakness, and kidney injury. If you suspect deficiency, testing is safer than guessing, especially if you take calcium, thiazide diuretics, or have kidney disease 8.

Zinc

Zinc is needed for normal reproductive function. In a small human study, severe zinc restriction lowered testosterone in young men, while zinc supplementation in marginally deficient older men increased testosterone, suggesting zinc helps most when deficiency is present 9.

High zinc intake can cause nausea, copper deficiency, anemia, nerve symptoms, and reduced immune function. Zinc can also interfere with some antibiotics and should not be treated as a harmless daily add-on 8.

Magnesium

Magnesium supports muscle and nerve function, sleep, and glucose metabolism. In one study of athletes and sedentary men, magnesium supplementation was associated with increases in free and total testosterone, especially in those doing taekwondo training, but this was not a large hypogonadism treatment trial 10.

Magnesium can cause diarrhea and cramping. People with kidney disease should be careful because magnesium can build up when kidney function is low 8.

Fenugreek

Fenugreek is an herb often marketed for libido and testosterone. In a 12-week randomized trial in healthy aging men, a fenugreek seed extract was linked with improvements in some androgen and sexual function measures, but studies are small and product-specific 11.

Fenugreek can cause stomach upset, maple-like body odor, and allergic reactions, especially in people with peanut or chickpea allergy. It may also affect blood sugar and interact with diabetes medications or blood thinners 6.

D-aspartic acid

D-aspartic acid is an amino acid involved in hormone signaling in animals and some human reproductive studies. In resistance-trained men, a 28-day randomized trial found that D-aspartic acid did not increase testosterone or improve strength compared with placebo 12.

Side effects are not as well studied as common vitamins and minerals. Because benefits are uncertain, taking large amounts without a clinician’s input is not a strong trade-off.

DHEA

DHEA is a hormone precursor made by the adrenal glands. In a 2-year randomized trial in older adults, DHEA increased some hormone levels but did not improve body composition, physical performance, insulin sensitivity, or quality of life in a clinically meaningful way 13.

DHEA is not a simple supplement. It can affect androgen and estrogen pathways and may worsen acne, hair loss, mood changes, hormone-sensitive conditions, or interact with other hormone therapies 13.

Tongkat ali and maca

Tongkat ali, also called Eurycoma longifolia, has small human studies suggesting possible effects on stress hormones, mood, or testosterone in select groups, but data quality varies and products differ 14. Maca is often marketed for libido, but a randomized trial found improved sexual desire without changes in testosterone or estradiol 15.

Both can cause side effects and product-quality concerns. Maca may cause stomach symptoms or sleep changes in some people, and tongkat ali products have raised concerns about contamination and dosing consistency in some markets, making source quality important 3, 6.

How can you naturally support testosterone through lifestyle?

Natural testosterone support works best when it fixes a real strain on the body, such as short sleep, heavy alcohol use, under-eating, untreated sleep apnea, or excess visceral fat. These steps are not a guaranteed way to raise testosterone, but they improve health and may help the hormone system work more normally.

Sleep

Sleep is one of the clearest lifestyle links. In a small study of healthy young men, restricting sleep to 5 hours per night for 1 week reduced daytime testosterone levels by 10% to 15% 16.

Snoring, gasping, morning headaches, and daytime sleepiness can point to sleep apnea, which is also linked with fatigue and sexual symptoms. If those signs are present, a sleep evaluation may be more useful than a supplement.

Resistance training

Resistance training can raise testosterone for a short time after workouts and supports muscle, insulin sensitivity, and body composition over time. The long-term testosterone effect varies, but strength training is still one of the highest-value habits for aging well 17.

More is not always better. Overtraining, poor sleep, very low calorie intake, and injury can push the body in the wrong direction, so training should be progressive and recoverable.

Body weight and nutrition

Obesity is strongly linked with lower testosterone, partly through insulin resistance, inflammation, sleep apnea, and changes in sex hormone binding globulin. Weight loss in men with obesity is associated with increases in testosterone, especially when weight loss is larger and sustained 18.

Nutrition matters too. Very low fat intake, crash dieting, and low protein can make training and hormones harder to support. A balanced pattern with enough protein, fiber, healthy fats, and micronutrients is more defensible than a “booster” stack.

Alcohol and stress

Heavy alcohol use can impair testicular function and the hormone signals that regulate testosterone. Chronic stress may also affect sleep, appetite, alcohol use, and libido, which can look like low testosterone even when the cause is broader 1.

Stress reduction does not have to be fancy. Consistent sleep timing, walking, therapy when needed, fewer late-night drinks, and treating anxiety or depression can all change the symptom picture.

Are testosterone boosters safe?

Testosterone booster safety depends on the ingredients, dose, health history, and product quality. The main risk is not just side effects; it is delaying the right diagnosis while taking a blend that may not contain what the label suggests.

  • Liver risk: Some bodybuilding and sexual-enhancement supplements have been linked with liver injury, especially when products contain hidden drugs or steroid-like compounds 6.
  • Drug interactions: Herbs and minerals can interact with blood thinners, diabetes medications, thyroid medications, antibiotics, sedatives, and hormone therapies 6, 8.
  • Fertility risk from hormone-like products: Testosterone and anabolic steroid exposure can suppress LH and FSH, which can lower sperm production 1.
  • Contamination and labeling risk: Dietary supplements are regulated differently than prescription drugs, and pre-market proof of efficacy is not required 3.
  • Missed diagnosis: Fatigue, low libido, and erectile dysfunction can come from diabetes, depression, sleep apnea, thyroid disease, medication effects, cardiovascular disease, or low testosterone 1, 2.

When should you see a doctor about low testosterone?

You should see a clinician when symptoms last more than a few weeks, affect sex, mood, energy, fertility, or strength, or show up with infertility, breast tenderness, testicular changes, fractures, or new erectile dysfunction. Morning testing is the usual starting point because testosterone has a daily rhythm 1, 2.

What a workup looks like

If TRT is being considered, guidelines recommend discussing fertility plans first because prescription testosterone can suppress sperm production. Monitoring may include testosterone levels, hematocrit, prostate risk review, symptom response, and side effects 1, 2.

Beyond testosterone: growth hormone and metabolic health

Some people who search for testosterone boosters are really trying to solve low energy, body composition changes, poor recovery, or age-related vitality. Testosterone is one possible part of that picture, but the growth hormone axis, sleep, insulin resistance, nutrition, and medications can also matter 1, 18.

That distinction matters because growth-hormone-axis care is not TRT. Sermorelin, a growth hormone releasing hormone analog, is designed to signal the pituitary growth hormone pathway; it is not testosterone and should not be used as a substitute for evaluating low testosterone.

  • If fertility matters to you, avoid testosterone-like products unless a clinician has reviewed the risks.

For many people, the best next step is not a supplement decision. It is a basic workup: morning testosterone, sleep review, medication review, metabolic labs, and a plan that matches the actual cause.
