Sirtuins and Aging: What Human Research Actually Shows

A plain-language guide to sirtuins, NAD+, resveratrol, and what longevity research can—and cannot—say today.

Sirtuins and Aging: What Human Research Actually Shows

Sirtuins are NAD+-dependent enzymes that help regulate DNA repair, metabolism, inflammation, mitochondrial function, and stress responses. Cell and animal studies link them to lifespan-related biology, but human evidence is limited. No sirtuin activator, NAD+ product, or supplement is proven to reverse aging or extend human lifespan. 1 2

What are sirtuins, and why are they linked to aging?

Sirtuins are a family of enzymes that use NAD+—nicotinamide adenine dinucleotide—to do their work. They are linked to aging because they influence cellular systems that often change with age, including energy metabolism, DNA repair, inflammation, oxidative stress, and mitochondrial function. 1 2

The seven mammalian sirtuins: SIRT1 through SIRT7

Humans have seven sirtuins: SIRT1, SIRT2, SIRT3, SIRT4, SIRT5, SIRT6, and SIRT7. They work in different parts of the cell, including the nucleus, cytoplasm, and mitochondria. That matters because DNA stability, energy production, and cell survival are managed in different cellular compartments. 1

Why sirtuins need NAD+ to work

Sirtuins use NAD+ as a cofactor, meaning they need it to carry out many chemical reactions. When cellular NAD+ availability changes, sirtuin signaling can change too. That biological fact does not prove that adding NAD+ through a supplement or medication produces a meaningful longevity benefit in people. 2 3

How sirtuins connect to DNA repair, inflammation, mitochondria, and metabolism

Sirtuins can influence proteins involved in DNA repair, FOXO stress-response signaling, PGC-1α and mitochondrial metabolism, NF-κB inflammatory signaling, p53-mediated cell responses, apoptosis, and cellular senescence. These are important pathways, but a pathway effect in a lab is not the same as a proven human health outcome. 1

How might sirtuins influence aging biology?

Sirtuins may help cells respond to damage and shifts in energy supply. The key limit is that most mechanistic evidence comes from cells and animals, where researchers can control genes, diets, and exposures far more tightly than in real life. 1 2

  • Cell evidence: Sirtuins can alter DNA-repair and stress-response pathways in cultured cells. This supports a mechanism, not a treatment claim. 1
  • Animal evidence: In model organisms and mice, changing sirtuin-related signaling has affected metabolic and stress-resistance measures. Findings vary by species, tissue, and intervention. 1 2
  • Human evidence: Researchers can measure NAD+-related metabolites, metabolic markers, and short-term safety, but there are no completed human trials showing that sirtuin activation extends lifespan. 3 4

This translation gap is central to geroscience. A treatment can improve a laboratory biomarker without changing symptoms, disease risk, function, healthspan, or lifespan. The National Institute on Aging notes that geroscience drug development needs clear populations, safety plans, and clinically meaningful outcomes. 7

What does human evidence say about sirtuins and longevity?

Human research supports the idea that NAD+-sirtuin biology is measurable and relevant to metabolism. But no human trial has shown a sirtuin activator extends lifespan or reverses biological aging in a clinically established way. 3 4

What is known from human clinical research

Human trials of NAD+ precursors such as nicotinamide riboside have mainly tested whether they raise NAD+-related blood measures and are tolerated over short periods. In a randomized trial involving healthy middle-aged and older adults, nicotinamide riboside increased NAD+ levels, but the study was not designed to test lifespan or prove broad anti-aging effects. 3

What observational studies can and cannot prove

Human observational studies can find links between diet, activity, metabolic health, and aging-related outcomes. They cannot prove that one pathway—such as SIRT1 activation—caused the outcome. People who exercise or follow healthier diets also differ in many other ways that affect health. 6

Why biomarkers are not the same as lifespan outcomes

A biomarker is a measurable sign of biology, such as a blood metabolite or gene-expression pattern. It can be useful for research, but it is not automatically a validated surrogate endpoint for living longer or avoiding disease. 7

What do animal and cell studies show about sirtuins?

Animal and cell studies make sirtuins scientifically interesting, especially for metabolism and stress responses. These findings are preclinical, however, and cannot establish that a person will live longer by changing sirtuin signaling. 1 2

Early research in yeast, worms, flies, and mice helped make sirtuins a major longevity research topic. Yet results have not always been consistent across models, and differences in dose, genetics, diet, and study design can change the outcome. Reviews of the field describe promise alongside major unanswered questions about translation to humans. 1 2

What supplements or interventions are said to activate sirtuins?

NAD+ precursors, resveratrol, calorie restriction, fasting patterns, and exercise are often discussed as sirtuin-related interventions. Exercise and calorie restriction have the strongest broad human health evidence, but that does not mean their benefits come only from sirtuins. 5 6

ApproachWhat the evidence showsImportant limitation
NAD+ precursorsHuman clinical studies show changes in NAD+-related measures and assess short-term safety. 3 4No proof of longer human lifespan or age reversal.
ResveratrolCell and animal research suggests effects involving PDE4, AMPK, and downstream signaling. 8It is not a proven direct SIRT1 activator or proven anti-aging treatment in humans.
Calorie restrictionA two-year human trial found calorie restriction affected metabolic and cardiometabolic measures. 5It did not test whether people lived longer.
ExerciseHuman evidence supports exercise for health and function; it also affects energy-sensing pathways. 6It cannot isolate sirtuins as the sole reason for benefit.

Resveratrol deserves special caution because marketing often simplifies its biology. A cell and animal study found that resveratrol's effects were linked to inhibition of phosphodiesterase enzymes, especially PDE4, and AMPK-related signaling rather than straightforward direct activation of SIRT1. 8

Considering NAD+ as part of your longevity plan?

At Chia, a licensed US provider can review your health questionnaire and discuss whether compounded NAD+ options fit your goals. Prescriptions require a medical evaluation and are not guaranteed. Medications are compounded by state-licensed US 503A pharmacies; compounded medications are not FDA-approved.

Can sirtuins reverse aging?

No. There is currently no proven way to reverse human aging through sirtuin activation. Sirtuins are part of aging biology, but improving a cellular pathway or biomarker is not proof of age reversal, disease prevention, or a longer life. 1 7

When marketing uses “reverse aging,” it may refer to changes in a lab marker, skin appearance, energy, or a proposed mechanism. Those are different outcomes, and none should be treated as evidence that a product has reset a person's biological age or lifespan. 7

How does NAD+ longevity care at Chia work?

At Chia, we offer NAD+ as a compounded injection, with plans currently starting at $179 per month, or a compounded nasal spray, with plans currently starting at $119 per month. Both forms require licensed-provider review, and compounded medications are not FDA-approved. Neither should be described as proven to activate sirtuins in a way that extends human lifespan. 2 3

What are practical, evidence-based ways to support healthy aging?

The strongest evidence for healthy aging still comes from basics that reduce major health risks over time. Regular physical activity, sleep, nutritious eating, preventive care, and managing blood pressure and glucose matter more than any single “longevity” pathway. 6 7

  • Aim for regular aerobic and strength-building activity appropriate for your health and abilities. 6
  • Work with a clinician on blood pressure, lipids, glucose, sleep, smoking, alcohol use, and recommended preventive care. 7
  • Protect social connection and cognitive health; aging well includes function and quality of life, not only laboratory values. 7
  • Treat longevity claims as questions to investigate, not promises. Our overview of human longevity research explains why human outcomes matter.

A careful conversation can separate an interesting mechanism from a useful treatment decision. Ask what outcome was actually studied, how long the study lasted, and whether the evidence came from people, animals, or cells. 7

  1. 1Is this claim based on human clinical research, human observation, animal research, or cell research?
  2. 2Did the study measure symptoms, function, disease outcomes, biomarkers, healthspan, or lifespan?
  3. 3What are the known side effects, medication interactions, and uncertainties?
  4. 4Is a licensed clinician reviewing my health history and current medications?
  5. 5If it is a compounded treatment, do I understand that it is not FDA-approved and that results are not established for compounded formulations?
